Resumen en inglés
In Colombia, the health system, according to reports from the World Bank, presented a 95% access to health index and a 72% quality of care by 2021. [1] While this increases the scope for continuous improvement, it is indicative of a strong and mature health system in terms of development and responsiveness that has been evidenced in pandemic emergencies. Currently, the Provision of Health Services obeys Law 1122 of 2007 and its regulatory decrees, whose main objective is to guarantee access and quality of services, optimize the use of resources, promote user-centered care approaches and achieve the financial sustainability of Public Health Service Providers. [2] The present work aims to determine the requirements for the hiring of outpatient services by brigades of specialists in Pediatrics, Internal Medicine, General Surgery, Orthopedics and Gynecology and Obstetrics, to facilitate access to health services for the population served in the State Social Enterprise (ESE) Hospital Nuestra Señora del Carmen de El Bagre Antioquia. Through the analysis carried out to the database of the institution, it is found that, for the specialty of Internal Medicine, on average per month, 233 consultations are requested, which makes it opportune to hire the specialty once a week or four a month. To the specialty of Gynecology, it is 182 applications, being viable the programming of three or four brigades per month. For the specialty of Orthopedics, the average is 93 requests, and for General Surgery, it is 55, making it possible to carry out one or two brigades for month of these two specialties. 13 An approach was made with the administration of the ESE Hospital Nuestra Señora del Carmen, in which the proposal was announced and it was agreed that, the ideal type of contract with each professional would be for the provision of services, the value to be agreed, taking into account the tariff manual of the Compulsory Traffic Accident Insurance (SOAT) current, in which the outpatient consultation of specialized medicine is established under the code CUP 39143, with a unit value of $ 57,666 COP (11.88 USD), so it is agreed to pay $ 50,000 COP (10.30 USD) for consultation to each professional, generating a profitability of $ 7,666 COP (1.58 USD) by consultation for the institution.
