Objetivo: Diseñar una herramienta informática para seguimiento farmacoterapéutico en una clínica de Montería. Metodología: la presente investigación es carácter descriptivo con investigación acción participativa. (IAP), de enfoque cualitativo porque no recolecta, analiza ni vincula datos medibles numéricamente en el estudio. Resultados: se realizó el diseño del software en base al guion interactivo y lúdico obteniendo una base de confiablidad. Conclusión: se logró Identificar las etapas del proceso de seguimiento farmacoterapéutico basado en la metodología DÁDER, en las cuales se definen explícitamente las etapas que componen el SFT y se describe paso a paso la forma más efectiva de aplicar la metodología.Pharmacotherapeutic follow-up, is one of the essential activities in pharmaceutical care, has been developing worldwide in recent times, includes detection, prevention and solution of problems with medications (PRM). Results: the software was designed based on the interactive and playful script, obtaining a reliability base. Conclusion: it was possible to identify the stages of the pharmacotherapeutic follow-up process based on the DÁDER methodology, in which the stages that make up the SFT are explicitly defined and the most effective way of applying the methodology is described step by step.
